Does your business file a UC-8 report or a Schedule C tax form?
Does your business have a Delaware business license?
Do your employees work at least 30 hours per week to qualify as full-time employees?
If you have fewer than 10 employees, do all employees want the insurance or have a legitimate reason to opt out (for example, already covered by a spouse)?
If you do not meet these criteria, then you are not eligible to offer group health insurance. Consider purchasing individual health insurance.
What do qualifying employers need to do?
Compile a list which includes: employee name, gender, date of birth, home state or zip code
and coverage level (e.g. single, parent/child, husband/wife, family or waiving coverage)
for each employee.
The carriers will require each participating employee to complete a confidential medical history form.
Employers must contribute at least 50 percent of the employee’s monthly premium costs.
